Jouw rol als Jobcoach Als jobcoach bij Base to Work help jij anderstaligen hun weg te vinden naar duurzaam werk en participatie in de Nederlandse maatschappij. Je werkzaamheden bestaan onder andere uit: - Het beheren van je eigen caseload binnen trajecten vanuit gemeenten en UWV - Het voeren van intakegesprekken en het opstellen van persoonlijke ontwikkel- en actieplannen - Het analyseren van competenties, belemmeringen en arbeidsmogelijkheden - Het verzorgen van individuele coaching en groepstrainingen - Het actief benaderen van werkgevers en realiseren van duurzame plaatsingen - Het monitoren van voortgang en rapporteren aan opdrachtgevers - Het bieden van jobcoaching en begeleiding op de werkvloer na plaatsing - Het zorgvuldig bijhouden van cliëntdossiers en trajectadministratie in Pinado; - Het opstellen van heldere voortgangsrapportages, evaluaties en eindrapportages voor gemeenten, UWV en andere opdrachtgevers. Je bent zichtbaar in de regio en bouwt actief aan relaties met gemeenten, werkgevers en samenwerkingspartners. Je werkt zelfstandig, maar maakt onderdeel uit van een betrokken en professioneel team.
