Je helpt volwassenen met een verstandelijke en/of psychiatrische beperking bij kleine én grote stappen naar meer zelfstandigheid. Soms is een wasje draaien of een boodschap doen al een hele mijlpaal. Met jouw aandacht en steun voelen zij zich gezien en durven ze verder te groeien. Wat ga je doen? Als assistent woonbegeleider ben je er op de momenten die ertoe doen. Je ondersteunt cliënten bij hun dagelijkse leven en helpt hen stap voor stap zelfstandiger te worden. Denk aan ondersteuning bij het huishouden, persoonlijke verzorging, ADL/HDL en het onderhouden van contacten met instanties. Maar jouw belangrijkste taak? De mens achter de hulpvraag zien. Je sluit aan bij de wensen, mogelijkheden en levensfase van iedere cliënt. Je biedt structuur, duidelijkheid en een luisterend oor. Je moedigt aan waar het kan, neemt over waar het moet en geeft vertrouwen wanneer iemand dat zelf even kwijt is. Je houdt goed in de gaten hoe het met cliënten gaat, signaleert veranderingen en bespreekt je observaties met de wooncoach A. Je werkt nauw samen met collega's, volgt gemaakte afspraken en weet wanneer je moet overleggen of hulp inschakelen. Geen dag is hetzelfde. Jouw doel wel: samen zorgen voor een veilige, stabiele woonomgeving waarin cliënten zich gezien voelen en kunnen groeien. Je werkt volgens een onregelmatig rooster met dag-, avond- en weekenddiensten. Als Assistent Woonbegeleider: - Je activeert en ondersteunt cliënten bij dagelijkse activiteiten vanuit het ondersteuningsplan. - Je ondersteunt bij verzorgende en huishoudelijke taken (ADL/HDL). - Je stimuleert zelfstandigheid en eigen regie. - Je signaleert veranderingen en houdt de voortgang van doelen in de gaten. - Je koppelt je bevindingen terug aan de woonbegelei...
