Je werkt zowel vanuit onze huisartsenpraktijk in Friesland als bij patiënten thuis in de regio. Deze afwisseling biedt je de kans om een sterke band op te bouwen met patiënten en een directe impact te maken op hun welzijn. Daarnaast krijg je de ruimte om samen met ons te ontdekken hoe de rol van 'behandelaar aan huis' verder kan worden ontwikkeld. Wat je gaat doen: - Consulten: voeren van consulten in onze huisartsenpraktijk in Leeuwarden, waarbij je patiënten ondersteunt en behandelt. - Visites: afleggen van huisbezoeken bij patiënten in de regio, waarbij je zorg op maat biedt in hun eigen omgeving. - Samenwerking: nauw samenwerken met huisartsen, wijkverpleegkundigen en andere zorgverleners om de zorg voor patiënten te coördineren. - Innovatie: bijdragen aan de verdere ontwikkeling van de functie en het verbeteren van onze digitale en persoonlijke zorgprocessen. - Flexibiliteit: zelfstandig plannen en uitvoeren van je werkzaamheden, met ondersteuning van een digitale dokterstas en een auto van de praktijk.
