Individueel begeleider groepspand en ambulant Wil jij een betekenisvolle rol spelen in het doorbreken van de cyclus van dak- en thuisloosheid of uitsluiting? Heb jij ervaring en passie voor het werken met kwetsbare individuen en complexe problematiek? Dan ben jij de versterking waar ons dynamische team naar op zoek is! Een leuke en afwisselende functie in de regio Lekstroom (Nieuwegein, IJsselstein, Houten, Vijfheerenlanden, Lopik), met de focus op een groepspand in Houten en ambulante cliënten in Nieuwegein. Begeleiding in het groepspand (Houten) In ons groepspand in Houten wonen 8 volwassen mannen voor wie dit de eerste opvang is na een periode van dakloosheid. Omdat dit hun eerste opvanglocatie is, moet er direct bij binnenkomst veel voor hen worden geregeld. Op deze locatie is er sprake van 24 uur begeleiding per week (wat neerkomt op gemiddeld 3 uur per persoon per week). Je voert de begeleiding op deze locatie samen met een vaste collega uit. Jullie zijn echter niet tegelijkertijd in het pand aanwezig, maar wisselen elkaar hierin af. Als begeleider ben je op jouw momenten aanwezig om de individuele trajecten nauwgezet te monitoren. Denk hierbij aan het opstellen van een begeleidingsplan, ondersteuning bij het aanvragen van een uitkering, het zoeken naar een passende dagbesteding en de communicatie met externe partners. Het uiteindelijke doel is altijd het toewerken naar een succesvolle uitstroom uit het groepspand naar een eigen woning. Daarnaast speel je een actieve rol in het positief beïnvloeden van de groepsdynamiek in het pand. Je voorziet de huisvergaderingen en ziet erop toe dat de leefregels worden nageleefd om een veilige en prettige woonomgeving te waarborgen. Ambulante begeleiding (Regio) Naast je werkzaamheden in het groepspand, begeleid je cliënten ambulant in de regio. Dit zijn volwassenen met een indicatie voor Beschermd Thuis of een regio-indicatie, die vaak vanuit de maatschappelijke opvang zijn doorgestro...
