- Begeleider ambulant beschermd thuis Hilversum Delen: Wil jij cliënten ondersteunen in hun eigen thuissituatie? Je biedt je intensieve begeleiding aan cliënten die zelfstandig wonen, maar daarbij ondersteuning nodig hebben. Je helpt cliënten bij het dagelijks leven, wonen en werken en sluit daarbij aan bij hun persoonlijke doelen en behoeften. Je biedt structuur, denkt mee, signaleert veranderingen en weet ook in onverwachte situaties de juiste ondersteuning te bieden. Je werkt vanuit het begeleidingsplan, rapporteert en onderhoudt contact met collega's en andere betrokkenen. Je werkt nauw samen met je team en krijgt volop de ruimte om mee te denken over deze manier van begeleiden. Geen dag is hetzelfde: juist de afwisseling tussen geplande begeleiding en onverwachte situaties maakt deze functie interess ant. Je werkt in wisseldiensten waarbij de roosteraars zo goed mogelijk rekening houden met jouw wensen. Het team werkt voor nu dagdiensten van 8:30-17:00, bereikbaarheidsdiensten van 17:00-22:15, op termijn zullen dit dag en avonddiensten worden. De weekenddiensten wissel je af met collega's, je zult ongeveer eens per maand een dagdienst of bereikbaarheidsdienst in het weekend hebben. Voor deze functie is het belangrijk dat je op de vrijdag werkzaam bent. Jouw werkplek Je bent werkzaam in het Team Beschermd Thuis Gooi & Vechtstreek. Je kunt vanuit huis of kantoor op huisbezoek gaan. Je begeleidt cliënten met een Beschermd Thuis beschikking (WMO) of Volledig Pakket Thuis indicatie (WLZ). Het doel in de begeleiding van onze cliënten, is het stabiliseren en waar mogelijk (met name voor cliënten met een WMO beschikking) gericht op verdere ontwikkeling op de zelfredzaamheid. Hierbij wordt op diverse leefgebieden als werk- en dagbesteding, wonen, sociale relaties en financiën begeleiding geboden.
