Jij zorgt ervoor. Zo werk jij Voor wie jij er als begeleider/ verpleegkundige beschermd wonen bent? Voor volwassenen met psychiatrische problematiek met een GGZ-indicatie (WMO/WLZ) die in een beschermende woonvorm begeleiding nodig hebben. Je bent het vaste aanspreekpunt voor de bewoners en werkt samen met je collega's aan de individuele herstelplannen van de cliënten. Jij ondersteunt en motiveert hen, zodat ze zo zelfstandig mogelijk kunnen deelnemen aan het sociaal-maatschappelijke leven. Waar nodig weet jij cliënten ook te begrenzen, vooral bij probleemgedrag. Een gedeelte van onze cliënten zijn adolescenten met autisme, ervaring en kennis van deze doelgroep is een pre. Vanuit jouw expertise en persoonlijke talenten geef jij vorm aan de functie begeleider/ verpleegkundige beschermd wonen. De belangrijkste taken en verantwoordelijkheden zijn voor jou bekend terrein. Wat de functie van begeleider/ verpleegkundige beschermd wonen bij GGz Centraal bijzonder maakt is: - het geven van individuele begeleiding aan cliënten, het deelnemen aan groepsactiviteiten, verantwoordelijk zijn voor herstelplannen, evaluaties en aanvragen van (her) indicaties. - het signaleren van hulpvragen en onderhouden van contact met het cliëntensysteem en daarin onder andere samenwerken met collega's vanuit FACT en het sociaal domein. - stimuleren van een gezond dag- en nachtritme en een veilige omgeving waar cliënten zich vrij voelen om te groeien. Je werkt in wisselende diensten tussen 07.15 uur en 22.45 uur, ook in weekenden. Er is een aparte nachtdienst.
