Dwarshoudt verbindt expertise uit de geestelijke gezondheidszorg en het VAPH. We bieden intensieve ondersteuning aan personen met een verstandelijke beperking en bijkomende ernstige psychiatrische problematiek, wanneer de zorgvraag de draagkracht van een reguliere context overstijgt.

We werken aan vaardigheden, emotieregulatie en alternatieve copingstrategieën, met als perspectief een duurzame doorstroom naar een passende wooncontext. Het traject start vóór een mogelijke opname en loopt door tijdens het residentiële verblijf en de voorbereiding van de vervolgcontext.

Jouw rol

Als begeleider ben je nabij in het contact met de gast en een spilfiguur in de dagelijkse afstemming. In het voortraject leer je de gast en diens context kennen, ondersteun je teams en bereid je een mogelijke opname praktisch mee voor. Tijdens het residentiële verblijf werk je mee in de leefgroep en help je het behandel- en begeleidingsplan dagelijks waarmaken.

Wat ga je doen?

1. Nabij begeleiden van gasten

  • Een vertrouwensrelatie opbouwen en onderhouden.
  • Ondersteunen bij emotieregulatie, dagstructuur, activiteiten en copingvaardigheden.
  • Een veilige, voorspelbare en op de gast afgestemde omgeving creëren.
  • Tijdens het residentiële traject actief meedraaien in de leefgroep.

2. Observeren en tijdig signaleren

  • Gedrag, spanning, interacties en ondersteuningsnoden gericht observeren.
  • De draagkracht van de omgeving en het team mee in beeld brengen.
  • Risico’s op escalatie herkennen en werken volgens gedeelde veiligheids- en signaleringsafspraken.
  • Observaties helder registreren en delen in het multidisciplinaire overleg.

3. Teams praktisch versterken

  • VAPH-teams in het voortraject en het team van Dwarshoudt ondersteunen op de werkvloer.
  • Structuur, begeleidingsstijl en concrete werkafspraken mee implementeren.
  • Samen uitproberen wat werkt en feedback geven over haalbaarheid en effect.
  • Collega’s on-the-job ondersteunen bij moeilijk verstaanbaar gedrag.

4. Verbinden en afstemmen

  • Actief deelnemen aan overleg met Dwarshoudt, VAPH-voorzieningen en andere partners.
  • Diagnostische en behandeladviezen samen met de orthopedagoog vertalen naar de dagelijkse praktijk.
  • Continuïteit bewaken tussen voortraject, opname en vervolgcontext.
  • De stem, voorkeuren en communicatienoden van de gast zichtbaar houden.
  • Je hebt een bachelor in een menswetenschappelijke richting, bijvoorbeeld orthopedagogie, sociaal werk of psychiatrische verpleegkunde.
  • Je hebt ervaring met of sterke affiniteit voor verstandelijke beperking en psychiatrische kwetsbaarheid.
  • Je bent sterk in relationeel en nabij werken, observeren en signaleren.
  • Je kunt structuur bieden en blijft afgestemd bij spanning of moeilijk verstaanbaar gedrag.
  • Je werkt praktisch, cliëntgericht, multidisciplinair en intersectoraal.
  • Je combineert zelfstandigheid met teamgericht werken en heldere verslaggeving.
  • Je bent flexibel, mobiel én in het bezig van een rijbewijs.
