Begeleider Gehandicaptenzorg LVB/VB | Gouda Wij zoeken een begleider voor 24 - 28 uur per week Wil jij mensen met een verstandelijke beperking helpen om zo zelfstandig mogelijk hun eigen leven te leiden? Bij Het Baken in Gouda begeleid je volwassenen van 25 tot 75 jaar in hun eigen appartement én in het gezamenlijke leven op de groep. Jouw rol Als Begeleider Gehandicaptenzorg bij Het Baken ondersteun je bewoners bij de dingen die voor hen belangrijk zijn. Daarbij neem je niet automatisch iets over. Je kijkt eerst: wat kan iemand zelf, waarbij is een zetje nodig en wanneer moet jij juist wat meer nabijheid of ondersteuning bieden? De ene bewoner heeft vooral behoefte aan structuur en duidelijkheid, terwijl een ander hulp nodig heeft bij praktische zaken of persoonlijke verzorging. Je leert de bewoners goed kennen en sluit jouw begeleiding aan op hun mogelijkheden en ondersteuningsvraag. Je: - helpt bewoners bij het aanbrengen van structuur en het organiseren van hun dagelijkse leven; - stimuleert zelfstandigheid en helpt bewoners om zoveel mogelijk hun eigen keuzes te maken; - ondersteunt waar nodig bij persoonlijke verzorging; - begeleidt bij praktische zaken zoals koken, het huishouden en sociale contacten; - hebt oog voor de groepsdynamiek en zorgt samen met collega's voor een prettige en veilige sfeer; - betrekt familie, vrienden en het netwerk waar dat passend is; - signaleert veranderingen en rapporteert belangrijke informatie natuurlijk ook in het ECD. Je werkt volgens het ondersteuningsplan, maar vooral mét de bewoner. Het doel is niet om alles voor iemand te regelen, maar om iemand te helpen zijn of haar eigen leven zo goed mogelijk vorm te geven. Over de locatie Het Baken is een woonlocatie van Philadelphia in een rustige woonwijk in Gouda . Hier wonen 16 volwassenen met een lichte tot matige verstandelijke beperking (LVB/VB).
