Je helpt bij dagelijkse begeleiding, ADL-ondersteuning en persoonlijke zorg, en zorgt voor een veilige, fijne en waardige oude dag. Geen werkdag is hetzelfde door de variatie in zorgvragen. Wat ga je doen? - Bieden van dagelijkse begeleiding, ADL-ondersteuning en persoonlijke zorg. - Ondersteunen van bewoners bij het behouden van vaardigheden of omgaan met afnemende mobiliteit/dementie. - Werken volgens het zorgplan, zorgvuldig rapporteren en afstemmen met collega's. - Bewoners begeleiden naar dagbesteding en ondersteunen bij verpleging en verzorging. - Als persoonlijk begeleider fungeren als eerste aanspreekpunt voor bewoners. - Contact onderhouden met familie, gedragskundige en manager om zorgplan up-to-date te houden. - Voor mbo4-professionals: coördineren van zorg, coachen van collega's en bewaken van kwaliteit.
