Locatie Op locatie Alfred Tepepad 2 in Driebergen wonen ouder wordende mensen met een verstandelijke beperking. Door het ouder worden kunnen hun zorg- en begeleidingsvragen veranderen. Zo kunnen medische vragen en veranderingen in gedrag toenemen. De locatie ligt centraal in een woonwijk. Jouw team Als begeleider Ouderen werk je samen met je collega's aan passende begeleiding en zorg. Samen kijk je goed naar veranderingen bij bewoners en stem je af wat nodig is. Je werkt vanuit belevingsgerichte zorg en vindt het belangrijk om bewoners structuur en duidelijkheid te bieden. Je werkt samen, reflecteert op je eigen handelen en neemt initiatief wanneer je veranderingen signaleert. Jouw uitdaging Je ondersteunt bewoners bij hun dagelijkse leven en sluit aan bij wat zij op dat moment nodig hebben. Je biedt structuur en helpt bij onder andere persoonlijke verzorging en andere dagelijkse activiteiten. Omdat bewoners ouder worden, kunnen hun behoeften veranderen. Je let daarom goed op signalen in gedrag, gezondheid en dagelijks functioneren. Je signaleert veranderingen en bespreekt deze met je collega's. Je werkt met het ECD , ondersteunt bij medicatie en hebt aandacht voor epilepsie . Je werkt methodisch en weet je begeleiding aan te passen wanneer de zorgvraag verandert. Je kijkt steeds naar wat iemand zelf kan en waar ondersteuning nodig is. Juist daarin komen liefde, lef en meesterschap samen. Je bent opmerkzaam, durft te signaleren en werkt samen met collega's aan passende begeleiding. Wat krijg je van ons?
