Als begeleider maak je écht het verschil en zorg je voor een veilige, warme omgeving. Functieomschrijving Als begeleider ondersteun je cliënten bij het dagelijks leven aan de hand van individuele begeleidingsplannen. Deze worden door de cliënten samen met de (coördinerend) begeleider opgesteld. Als begeleider heb je bij ons je eigen caseload. Samen met een coördinerend begeleider ben je verantwoordelijk voor de begeleidingsplannen van een aantal bewoners. Je levert een bijdrage aan een goede leefsfeer, de veiligheid en het welbevinden van cliënten. Je signaleert als begeleider en rapporteert knelpunten in het kader van de dagelijkse zorgverlening en gezondheidstoestand van de cliënt en je ziet toe op medicatieverstrekking. Verder ondersteun je de cliënt zo nodig bij het beheer van financiële en materiële middelen. Je begeleidt groepsgesprekken en ziet erop toe dat cliënten op een fijne manier samenleven. Ook stimuleer je de zelfredzaamheid van cliënten. Wij zijn opzoek naar een begeleider die 24 tot 32 uur (in overleg) per week wilt werken. Vanaf september 2026 bevindt de lorentzweg zich tijdelijk in het oude tergooi ziekenhuis in blaricum, dit omdat hun eigen pand wordt gernoveerd. Jouw werkplek De BW+ is onderdeel van beschermde woonvorm de Lorentzweg in Bussum. We bieden begeleiding en ondersteuning in de ADL aan 26 bewoners vanaf ongeveer 70 jaar. De bewoners hebben een psychiatrische achtergrond, een aantal hebben een somatische zorgvraag. Je komt te werken binnen een betrokken en ervaren team van ongeveer 26 medewerkers bestaande uit (coördinerend) begeleiders, meewerkend begeleiders, huishoudelijke ondersteuners en activiteitenbegeleiders.
