Je werkt in een hypermodern gebouw dat is ontworpen voor comfort, efficiëntie en samenwerking, met innovatieve voorzieningen, slimme technologieën en veel aandacht voor rust en ruimte. De afdeling Kliniek Ouderen en Middelengebruik richt zich op ouderen (65+ doelgroep) met verslavingsproblematiek. Met plek voor detox cliënten en behandelbedden zijn we een kleinschalige afdeling met 16 bedden. Als begeleider werk je in een multidisciplinair team aan herstel, stabiliteit en kwaliteit van leven van onze cliënten. Een uitdagende functie waarin je brede ervaring opdoet binnen de verslavings- en psychiatrische zorg Als social worker voor de afdeling ouderen en middelengebruik maak je deel uit van een team dat elke dag topzorg levert. Het multidisciplinaire team waar je onderdeel van zal worden bestaat uit: regiebehandelaar, psychiater, arts, behandelaren voor het moduleprogramma, verpleegkundigen /begeleiders, leerling verpleegkundige/HBO Social Work en stagiaires. De werkzaamheden: - je begeleidt ouderen met een verslavingsprobleem tijdens hun opname op de afdeling, op basis van het opgestelde behandelplan. Daarbij werk je nauw samen binnen een multidisciplinair team en met de polikliniek; - je rapporteert zorgvuldig, draagt bij aan de dossiervorming en helpt bij het opstellen van het signaleringsplan; - als persoonlijk begeleider ben je het eerste aanspreekpunt voor een aantal ouderen tijdens hun opname; - samen met het behandelteam geef je vorm en inhoud aan de behandeling en...
