Bij Prisma is elke dag een nieuw avontuur. Als begeleider / persoonlijk begeleider op onze woonvoorziening Buurstede 134 ondersteun je 21 bewoners met een lichte verstandelijke beperking (lvb). De leeftijden variëren van eind 20 tot eind 70 jaar, wat jouw werkdag ontzettend divers maakt. Een dag uit het leven van een begeleider / persoonlijk begeleider We zoeken voor Buurstede 134 in Oosterhout een ervaren begeleider / persoonlijk begeleider die enthousiast wordt van deze mooie groep bewoners! Bij de begeleiding van de cliënten van Buurstede 134 draait het om verbinding, vertrouwen en nabijheid, waarbij we de eigen regie en autonomie van de cliënt respecteren. We staan naast de cliënt en bewegen indien gewenst mee, werkend via de presentie-gerichte benadering. Sommige cliënten laten complexer gedrag zien. Er is daarnaast sprake van minimale adl zorg. Ondersteuning vindt plaats op het appartement van de cliënt of vanuit de gezamenlijke huiskamer. Naast de dagelijkse zorg en begeleiding op de groep, ben je als persoonlijk begeleider verantwoordelijk voor een aantal cliënten. Je bent hun vaste aanspreekpunt en behartigt hun belangen. Je stelt samen met de cliënt het ondersteuningsplan op, voert dit uit en bewaakt de voortgang. Ook neem je deel aan multidisciplinaire overleggen en stem je regelmatig af met naasten, behandelaren en andere betrokkenen. Zo zorg je ervoor dat de begeleiding goed aansluit bij wat de cliënt nodig heeft en dat afspraken worden nagekomen. Je houdt het overzicht, neemt initiatief en zorgt voor verbinding tussen alle betrokkenen rondom de cliënt. Wat bieden wij? Onze medewerkers zijn het hart van Prisma.
