Vind je het mooi om ouder wordende mensen met een verstandelijke beperking te begeleiden en samen met collega's de beste zorg te bieden? Dan is deze functie op woongroep De Moerbei iets voor jou. Dit ga je doen: Als Begeleider wonen (mbo niveau 4) begeleid en ondersteun je cliënten met een verstandelijke beperking in het dagelijks leven. Je stimuleert hen om zoveel mogelijk zelf te blijven doen en biedt ondersteuning waar dat nodig is. Van jou verwachten we meer dan alleen dagelijkse begeleiding. Je levert een actieve bijdrage aan het opstellen, uitvoeren en evalueren van de begeleidingsplannen. Je signaleert veranderingen in het functioneren van cliënten, denkt mee over passende ondersteuning en werkt nauw samen met de coördinator zorg, behandelaren en collega's. Samen met het team draag je verantwoordelijkheid voor de kwaliteit van zorg en de dagelijkse gang van zaken op de woning. Over De Moerbei Je gaat werken op De Moerbei, een woning voor ouder wordende cliënten met een verstandelijke beperking. De woning biedt plaats aan zes tot acht cliënten die intensieve begeleiding en verzorging nodig hebben. Bij meerdere cliënten is sprake van lichamelijke achteruitgang of beginnende dementie. Daarom zijn rust, herkenbare dagstructuren en een vertrouwde leefomgeving belangrijk. Je ondersteunt cliënten bij de dagelijkse verzorging (ADL), bent alert op veranderingen in gezondheid en gedrag en sluit aan bij wat iemand nog zelf kan.
