Wil jij met bewegen écht iets betekenen voor kwetsbare ouderen? Dignita is op zoek naar een enthousiaste bewegingsagoog voor 32 uur per week die ons multidisciplinaire team komt versterken. Als bewegingsagoog bij Dignita werk je met kwetsbare ouderen met uiteenlopende zorgvragen. Je begeleidt cliënten bij bewegen en draagt daarmee bij aan het behouden en verbeteren van hun mobiliteit, zelfstandigheid, welzijn en kwaliteit van leven. Je werkt op verschillende kleinschalige woonzorglocaties en verpleeghuislocaties én binnen de eerstelijnszorg. Daarbij werk je nauw samen met onze fysiotherapeuten en andere paramedische disciplines. Wat ga je doen? Als bewegingsagoog: • begeleid je individuele cliënten en groepen bij passende beweegactiviteiten; • stimuleer je ouderen om zo actief mogelijk te blijven en sluit je aan bij hun mogelijkheden en persoonlijke doelen; • ondersteun je de fysiotherapeut bij het uitvoeren van beweegzorg; • signaleer je veranderingen in het functioneren van cliënten en stem je hierover af met de betrokken behandelaren; • werk je samen met fysiotherapeuten, ergotherapeuten, logopedisten en andere zorgprofessionals; • denk je actief mee over het verder ontwikkelen van ons beweegaanbod; • werk je zowel intramuraal als in de eerste lijn. Onze cliënten hebben regelmatig te maken met complexe en chronische aandoeningen, waaronder Parkinson(isme), dementie, NAH, CVA en Multiple Sclerose. Wat vragen wij van jou? • Je hebt een afgeronde MBO-opleiding Bewegingsagogie. • Je hebt affiniteit met kwetsbare ouderen; ervaring binnen de ouderenzorg is een pré. • Je vindt het leuk om zowel individueel als met groepen te werken. • Je kunt zelfstandig werken, maar zoekt ook actief de samenwerking binnen een multidisciplinair team. • Je bent communicatief sterk, betrokken, flexibel en cliëntgericht. • Je kunt goed plannen en organiseren en werkt nauwkeurig. • Je kunt werken met digitale systemen, waaronde...
