En heb je ervaring binnen het sociaal domein met doelgroepen zoals NAH, autisme, psychiatrie en/of LVB? Dan is deze functie als Buurtteammedewerker bij Buurtteams Volwassenen Nijmegen interessant voor jou. Als buurtteammedewerker ben je een vast aanspreekpunt voor inwoners van Nijmegen die tijdelijk ondersteuning nodig hebben. Je werkt dichtbij de inwoners, in de wijk en vanuit een integrale blik op welzijn, zorg, werk en inkomen. Je begeleidt inwoners bij het oplossen van uiteenlopende hulpvragen en ondersteunt hen bij het duurzaam verbeteren van hun situatie. Daarbij kijk je altijd naar wat iemand zelf kan, welke ondersteuning het sociale netwerk kan bieden en wanneer aanvullende hulp nodig is.
