* eng mit den Einsatzkoordinator:innen zusammenarbeiten

* die Klient:innenbedürfnisse erheben und die erforderliche Pflegeplanung analog des Pflegeprozesses lt. GuKG erstellen

* den Einsatzplan mit unseren Klient:innen vereinbaren

* die fachliche Ansprechperson für unsere Kolleg:innen der Hauskrankenpflege (Heimhilfen, Pflegeassistenzberufe, DGKP) sein

* die Schnittstelle im interdisziplinären Betreuungsteam sein, um eine qualitativ hochwertige Versorgung zu gewährleisten

MITBRINGEN SOLLTEST DU...

* Diplom der Gesundheits- und Krankenpflege

* Berufserfahrung (idealerweise im mobilen Pflege- & Betreuungsbereich wünschenswert)
