Casemanager Gebiedsteam Utrecht Zuid - GGZ-agoog / Verpleegkundige Gebiedsteam Utrecht Zuid verleent specialistische ambulante behandeling aan mensen die een zorgvraag hebben op verschillende levensgebieden zoals wonen, werken, zingeving, gezondheid, zingeving, financiën en sociale contacten. Wij werken volgens de F-ACT methode: ambulante, wijkgerichte zorg: rehabilitatie, crisiszorg, behandeling en hulp bij herstel door verschillende mensen die in één team samenwerken. We werken intensief samen met Lister en tevens met de gezondheidscentra, de buurtteams van de gemeente, diverse ketenpartners en naastbetrokkenen. Daardoor kunnen we snel op- en afschalen in de intensiteit van zorg. Wat ga je doen? Als casemanager maak je deel uit van een multidisciplinair team, waarin iedereen vanuit de eigen expertise bijdraagt aan één gezamenlijk doel: goede zorg voor onze patiënten. In nauwe afstemming met de andere behandelaren heb jij de regie over de begeleiding en behandeling van jouw eigen caseload. Je ziet patiënten bij hen thuis in de wijken Hoograven en Lunetten of op onze locatie aan het Furkaplateau 15 in Utrecht. Je werk is afwisselend en je bent betrokken bij verschillende onderdelen van het behandelproces. Zo voer je intakes uit, werk je samen met collega's in crisissituaties en bied je systeem- en klachtgerichte behandelingen. Daarbij stem je steeds af wat een patiënt nodig heeft en werk je samen met het team aan passende zorg. Wat hebben we jou te bieden? Ontwikkeling staat bij Altrecht centraal.
