32- 36 uur Heb jij als verpleegkundige hart voor cliënten met ernstige psychiatrische aandoeningen én verslavingsproblematiek? Als verpleegkundig casemanager bij het FACT-team word jij ingeschakeld bij psychiatrische en somatische vraagstukken, en ben je casemanager van je eigen cliënten. Je bent intensief betrokken bij het herstel van cliënten waarbij vaak sprake is van een dubbele diagnose (EPA en verslaving). Hierbij staan stabilisatie, schadebeperking en veiligheid centraal. Jij combineert jouw verpleegkundige blik feilloos met herstelondersteunende zorg. In verbinding met het multidisciplinaire team lever jij kwalitatief hoogwaardige en outreachende zorg. Aandacht voor persoonlijke groei Als casemanager ben je verantwoordelijk voor de zorgcoördinatie van jouw cliënten. Je legt huisbezoeken af of spreekt cliënten op de FACT-locatie. Dagelijks worden alle cliënten in het team besproken tijdens de dagstart met het FACT-bord, maar je overlegt ook met de regiebehandelaar en collega's over de zorg voor jouw cliënten. Jij bent het aanspreekpunt voor jouw cliënten en daarnaast ook aandachtsfunctionaris voor jouw inhoudelijke expertise. Met een creatieve en herstelgerichte blik maak je het verschil voor cliënten, ook wanneer hun situatie complex is. In het multidisciplinaire team kijkt iedereen vanuit zijn eigen expertise, waardoor elke cliënt de beste zorg krijgt. Wat je verder gaat doen: - Je signaleert veranderingen in het toestandsbeeld (zowel psychiatrisch, somatisch als op het gebied van middelengebruik) en anticipeert hierop. - Je zet interventies in gericht op verslavingszorg, zoals motiverende gespreksvoering en leefstijltraining. - Je bent verantwoordelijk voor verpleegkundige handelingen, waaronder toedienen van (depot)medicatie en somatische screening. - Je onderhoudt nauw contact met naasten, regiebehandelaren, netwerkpartners en maatschappelijke organisaties. - Je komt terecht in een multidisciplinair team dat passie he...
