Casemanager bij Gebiedsteam Zeist Kern Wil jij als verpleegkundige, SPV of GGZ-agoog naast mensen staan en bijdragen aan hun herstel? Als casemanager bij Gebiedsteam Zeist Kern begeleid je mensen met een ernstige psychiatrische aandoening (EPA) en complexe zorgvragen. Je werkt ambulant binnen Zeist, krijgt veel eigen verantwoordelijkheid en bouwt langdurige relaties op met cliënten in hun eigen leefomgeving. Vanuit ons kantoor in Zeist ben je veel op pad. Je bezoekt cliënten thuis, spreekt hen op locatie of online en werkt nauw samen met collega's. Omdat je binnen één werkgebied werkt, leer je de wijken, het netwerk en de samenwerkingspartners goed kennen. Veel collega's pakken voor huisbezoeken de fiets. De ene dag voer je een intake, de andere dag denk je mee over een crisissituatie of bespreek je met collega's wat een cliënt nodig heeft. Geen dag is hetzelfde, maar je weet wel voor wie en voor welk gebied je werkt. Wat maakt werken bij ons bijzonder? Gebiedsteam Zeist Kern is een betrokken team collega's. We kennen elkaar goed, zoeken elkaar makkelijk op en nemen samen verantwoordelijkheid voor onze cliënten. We werken volgens de FACT-methodiek en bieden zorg zoveel mogelijk in de leefomgeving van de cliënt. Daarbij vinden we het belangrijk dat cliënten zoveel mogelijk de regie houden over hun eigen leven. Je werkt samen met psychiaters, verpleegkundig specialisten, (GZ-)psychologen, verpleegkundigen, GGZ-agogen, ervaringsdeskundigen en andere disciplines. Ieder brengt zijn eigen expertise mee en samen kijken we wat nodig is. Wat ga je doen? Als casemanager ben je een belangrijk aanspreekpunt voor cliënten en werk je samen met hen aan herstel, stabiliteit en kwaliteit van leven. Je werkzaamheden bestaan onder andere uit: - Je begeleidt cliënten met een ernstige psychiatrische aandoening en complexe zorgvragen. - Je voert gesprekken, intakes en behandel- en evaluatiegesprekken. - Je gaat op huisbezoek en spreekt cliënt...
