Casemanager complexe scheidingen Weert 2 plekken | 24 tot 40 uur per week | loondienst | cao Jeugdzorg Als casemanager bij aXiehuis in Eindhoven begeleid je trajecten voor kinderen en ouders bij complexe scheidingen. Je maakt afspraken helder, houdt overzicht en zorgt dat betrokken professionals goed kunnen samenwerken. Je spreekt met ouders, stemt af met collega's en verwijzers en kijkt wat een gezin nodig heeft. Je luistert zorgvuldig en durft aan te geven welke vervolgstappen nodig zijn. Wat ga je doen? Van intake tot eindverslag ben jij het vaste aanspreekpunt. Je zit geregeld met ouders aan tafel en zorgt dat collega's gericht kunnen werken. - Je maakt doelen en afspraken concreet en bewaakt de voortgang van een traject. - Je stemt af met omgangsbegeleiders, kindercoaches, ouderbemiddelaars, gemeenten en andere betrokkenen. - Je bespreekt signalen over veiligheid of ontwikkeling op tijd en bepaalt samen met collega's wat nodig is. - Je zorgt voor heldere evaluaties en eindverslagen. Feitelijk, leesbaar en binnen de afgesproken termijn. Wat heb je nodig? Je hoeft niet overal meteen een antwoord op te hebben. Wel moet je kunnen doorvragen, keuzes uitleggen en overzicht houden wanneer het druk wordt. - Een relevante hbo-opleiding, bijvoorbeeld Social Work of Pedagogiek. - Ervaring met casusregie of jeugdhulp. Kennis van complexe scheidingen is een voordeel. - Je kunt neutraal luisteren, duidelijk begrenzen en zorgvuldig rapporteren. - Een rijbewijs en een auto. Incidenteel werken op zaterdag is voor jou mogelijk. - Een SKJ-registratie, of de bereidheid samen te bepalen of die voor jouw werkzaamheden nodig en haalbaar is. Wat bieden wij? Ook wanneer een casus lastig is, hoef je het niet alleen uit te zoeken. - Een arbeidsovereenkomst voor 24 tot 40 uur per week. - € 3.055,29 tot € 5.940,00 bruto per maand op basis van 36 uur, cao Jeugdzorg.
