- Casemanager Dementie Wijkverpleging Casemanager Dementie Dementie is een ingrijpende ziekte, waarbij het omgaan met dementie en het vinden van de weg in alle wetten, regels en zorgmogelijkheden ingewikkeld is. Ben jij in staat om cliënten en hun naasten op die juiste weg te helpen? Weet jij echt verschil te maken en ben je creatief in het zoeken naar passende oplossingen. Dan is deze baan als casemanager dementie jou op het lijf geschreven! In het kort: - 24 uur - Bepaalde tijd met intentie tot vast - Hattem, Wijkverpleging Uitdagend Ruimte voor nieuwe ideeën Betekenisvol Wat ga je doen? Als casemanager ben je een onafhankelijke en vaste begeleider voor mensen met dementie én hun naasten. Je begeleidt de client om zo lang mogelijk thuis te blijven wonen in een vertrouwde omgeving en indien noodzakelijk ook naar een opname. Je coördineert, informeert, begeleidt, denkt mee, adviseert, helpt keuzes te maken en regelt zorg. Maar ook bied je een luisterend oor en emotionele begeleiding, zowel aan de cliënt met dementie als aan diens naasten. Je helpt om te gaan met de ziekte en met de gevolgen ervan in het dagelijks leven. Je kijkt hierin verder dan alleen de gestelde diagnose. Soms past de voor de hand liggende oplossing niet. Jij zoekt dan verder naar een oplossing waar anderen misschien niet aan gedacht hebben. De best passende oplossing waardoor de client opbloeit, dat is waar jij het voor doet! Daarnaast verwerk je de gemaakte afspraken in het zorgplan dat je samen bespreekt met de client en/of de naasten. Je zorgt voor een goede afstemming tussen de verschillende zorgverleners, zowel informeel als formeel, je versterkt het netwerk rondom de client en je bent in staat om familiegesprekken te voeren. Waar ga je werken? De casemanagers werken in Zwolle, Zwartewaterland, Kampen en Hattem/Heerde. De huidige vacature is voor het werkgebied Hattem en Heerde. Wie ben jij?
