Je bent er vanaf het allereerste moment en blijft betrokken gedurende het hele ziekteproces. Met jouw deskundigheid, rust en betrokkenheid bied je houvast in een onzekere periode. Jij bent het vertrouwde gezicht, de vaste sparringpartner en de verbindende schakel tussen alle betrokkenen. Omdat de lijnen kort zijn, heb je direct contact met alle schakels binnen én buiten de keten. Zo kun je snel handelen en écht het verschil maken. Soms begeleid je een cliënt al vóórdat er een diagnose is gesteld. Samen stel je een ondersteuningsplan op, met één helder doel: dat de cliënt zo lang mogelijk thuis kan blijven wonen - veilig, in vrijheid en met zoveel mogelijk kwaliteit van leven. Je kijkt verder dan de ziekte alleen. Je luistert naar wat iemand belangrijk vindt, speelt in op levensvragen en helpt bij het vinden van zinvolle activiteiten en het behouden van waardevolle contacten.
