- HWW Zorg - Den Haag ( Zuid-Holland ) HWW Zorg - Vast contract - Voltijds - Minstens 2 jaar ervaring - Hoger beroepsonderwijs - Casemanager Dementie Maak het mee, bij HWW zorg Vacature Casemanager (24 tot 36 uur per week) Bij HWW zorg zetten wij ons elke dag in voor persoonsgerichte zorg. Als Casemanager Dementie speel jij een cruciale rol in de begeleiding van mensen met dementie én hun mantelzorgers. Je bent de verbindende schakel in het zorgproces, houdt het overzicht en zorgt ervoor dat niemand er alleen voor staat. Wat ga je doen/over de functie: Als Casemanager Dementie ben je verantwoordelijk voor het coördineren van zorg en ondersteuning aan cliënten met dementie. Daarbij begeleid je ook hun mantelzorgers, zowel praktisch als emotioneel. Je bent een onafhankelijk adviseur die zorgt voor een naadloze afstemming tussen alle betrokken zorgverleners. Je: - Voert intake- en begeleidingsgesprekken met cliënten en mantelzorgers. - Stelt zorgplannen op, voert deze uit en evalueert ze regelmatig. - Voorkomt proactief problemen in het leefklimaat van cliënten. - Neemt deel aan multidisciplinaire overleggen (MDO's) en werkt samen met behandelaren, begeleiders en externe hulpverleners. - Levert een actieve bijdrage aan werkgroepen en deskundigheidsbevordering. Het basissalaris bedraagt maximaal € 5.413,38 (FWG 55), afhankelijk van jouw kennis en ervaring voor een werkweek van 36 uur. Maar daar blijft het niet bij. Verder bieden wij uitstekende secundaire arbeidsvoorwaarden. Naast een volledige 13e maand, ontvang je 8% vakantiegeld en heb je recht op maar liefst 238 verlofuren per jaar.
