Kom dan werken voor Skills Centrum Nederland! Werk waar je trots op kan zijn! Als Jeugd- en gezinsprofessional heb je in deze functie twee taken. Enerzijds ben je de casemanager van gezinnen, die kortdurend betrokken is. Je gaat op huisbezoek bij gezinnen waar soms complexe vragen spelen, waar onveiligheid ervaren kan worden en waar ouders of jongeren soms niet weten hoe ze de situatie positief kunnen veranderen. Jouw expertise ligt bij jouw gespreksvoering, waarmee jij goed in staat bent om de behoefte van elk gezinslid in kaart te brengen. Daarnaast kun je een goede analyse maken van wat er nodig is en wat het gezin zelf kan. Anderzijds ben jij het eerste aanspreekpunt van het jeugd- en gezinsteam, inwoners en samenwerkingspartners. Dit is een dynamische functie, waarbij je de telefoon opneemt, inkomende mails beantwoordt en mensen met meer algemene vragen de juiste weg kunt wijzen. Je kunt goed doorvragen en bent goed in staat de urgentie van een binnenkomende vraag te wegen. Dit ga je doen: Als lid van ons toegewijde team ben je verantwoordelijk voor het beantwoorden van diverse vragen over jeugdhulp, zowel van inwoners als zorgaanbieders. Je speelt een cruciale rol m.b.t. het oppakken en afhandelen van zorgmeldingen, waarbij je grondig onderzoekt, analyseert en indien nodig doorverwijst naar de juiste hulpverlening. Dit kan variëren van snelle afhandelingen tot intensieve betrokkenheid gedurende ongeveer drie maanden. Daarnaast ben je de schakel tussen onze samenwerkingspartners en de inwoners, en verdeel je nieuwe aanvragen onder de casemanagers.
