En wil je meebouwen aan toekomstgerichte, netwerkgerichte ggz binnen de poli van GGZ Rivierduinen? Dan is dit jouw kans. Op de locatie Voorhout werk je als casemanager samen met een betrokken team aan het behandelen van mensen met uiteenlopende en vaak meervoudige psychiatrische problemen. Vanuit een herstelgerichte visie help je mensen in hun thuissituatie of op locatie om hun leven weer zo zelfstandig en betekenisvol mogelijk vorm te geven. Je komt terecht in een dynamische werkomgeving waar we in januari starten met Rivierduinen Verleggen . Een beweging waarin we onze krachten bundelen en poliklinische zorg, FACT-zorg en ouderenpsychiatrie samenbrengen binnen één herstelgerichte netwerkstructuur. Dit betekent: samenwerken over grenzen heen, leren van verschillende doelgroepen en actief meebouwen aan de toekomst van onze zorg. Je bent verantwoordelijk voor de zorgcoördinatie van jouw caseload en speelt een centrale rol in het herstelproces van cliënten. Binnen Verleggen staat gezamenlijke besluitvorming centraal. Je werkt samen met cliënten én hun netwerk aan hersteldoelen en stemt zorg zorgvuldig af, binnen en buiten de ggz. Specialistische kennis blijft altijd beschikbaar via onze expertisenetwerken, waarbij we werken volgens het principe: meedenken - meedoen - overnemen.
