Wil jij werken aan passende ondersteuning voor inwoners in Nieuwkoop? Voor Tom in de buurt zoeken we een Casemanager Wmo die dichtbij inwoners staat en samen met hen kijkt wat nodig is. Max. € 4.822,50 (o.b.v. 36 uur) Plaats Omgeving Nieuwkoop Locatie / afdeling Regio Nieuwkoop Uren 24 uur Wie is TOM in de buurt? TOM in de Buurt is een samenwerkingsverband van vijf hulpverlenende organisaties: stichting ActiVite, De Binnenvest, Gemiva, Kwadraad en WMO Maatwerk. Samen verzorgen zij de Wmo-hulp voor de gemeenten Alphen aan den Rijn, Kaag en Braassem en Nieuwkoop. TOM in de Buurt ondersteunt volwassen inwoners met sociaal-maatschappelijke vragen. We bieden informatie, advies en ondersteuning, zoveel mogelijk dichtbij de inwoner: thuis, digitaal, telefonisch, in een buurthuis of op een locatie van TOM in de Buurt. Daarbij kijken we samen met de inwoner wat nodig is en betrekken we ook het netwerk en de mogelijkheden in de omgeving. De ondersteuning kan bestaan uit een Wmo-voorziening, training, dagbesteding, begeleiding voor mensen met een licht verstandelijke beperking (LVB), maatschappelijk werk of andere passende ondersteuning. Een goede kennis van de sociale kaart is hierin belangrijk, zodat inwoners snel en gericht geholpen kunnen worden. Samen met de host ben jij het gezicht van de toegang. Jullie maken deel uit van een zelforganiserend wijkteam, bestaande uit wijkcoaches, mantelzorgadviseurs en ervaringsdeskundigen. Elk wijkteam heeft een manager die faciliteert, ondersteunt en stuurt op de opdracht. Binnen TOM werken we volgens 1 huishouden, 1 plan, 1 regisseur (1H1P1R), Positieve Gezondheid en systeem- en ervaringsgericht werken. We werken vanuit een ondersteuningsmix: 1-op-1 begeleiding waar het moet en digitaal of groepsgericht waar mogelijk.
