Ben je stressbestendig en houd je van snel schakelen? Dan zijn we op zoek naar jou! Functiebeschrijving Als CAT-medewerker binnen het Stevig Lokaal Team heb je een veelzijdige en dynamische functie. Je zorgt ervoor dat jeugdigen en gezinnen tijdig de ondersteuning ontvangen die past bij hun situatie. Je brengt hulpvragen, context, urgentie en benodigde ondersteuning integraal in beeld en draagt zorg voor een zorgvuldige toegang en triage. Je organiseert waar nodig consultatie en multidisciplinaire afstemming en bewaakt de samenhang en voortgang wanneer de regie bij het Centraal Aanmeld- en Triage Team (CAT) is belegd. Daarbij werk je nauw samen met professionals uit verschillende organisaties en domeinen. Je weet overzicht te bewaren, zorgvuldig afwegingen te maken en passende ondersteuning te organiseren. Vanuit een gezamenlijke professionele grondhouding draag je bij aan tijdige, samenhangende en passende hulp voor jeugdigen en gezinnen. Wat ga je doen Binnen de Stevige Lokale Teams werken professionals vanuit verschillende organisaties samen aan één gezamenlijke opdracht: jeugdigen en gezinnen tijdig, passend en zo dichtbij mogelijk ondersteunen. Als CAT-medewerker ben je een belangrijke schakel binnen deze samenwerking en werk je nauw samen met gebiedsteams, netwerkpartners en andere betrokken professionals. • Je verzorgt toegang en triage en brengt hulpvraag, context, urgentie en benodigde ondersteuning integraal in beeld. • Je maakt bij nieuwe aanmeldingen dossiers aan en legt afwegingen en besluiten transparant en navolgbaar vast. • Je organiseert consultatie en multidisciplinaire afstemming wanneer dat nodig is. • Je zorgt voor passende toeleiding naar ondersteuning binnen of buiten het Stevig Lokaal Team. • Je bewaakt een uniforme toepassing van de afgesproken werkwi...
