Als cliëntadviseur ben je een belangrijke schakel tussen cliënten, hun netwerk en onze zorglocaties. Je begeleidt het proces vanaf de eerste aanmelding tot aan een passend zorgaanbod en draagt bij aan een zorgvuldig en toekomstbestendig zorgtraject. Wat ga je doen? Als cliëntadviseur ben je het eerste aanspreekpunt voor cliënten, ouders, wettelijk vertegenwoordigers, verwijzers en andere betrokkenen. Je luistert naar de zorgvraag, brengt de situatie zorgvuldig in kaart en onderzoekt welke ondersteuning binnen Siloah het beste aansluit. Vervolgens vertaal je de zorgvraag naar de verschillende zorglocaties. Je begeleidt het gehele plaatsingsproces en zorgt ervoor dat cliënten en hun vertegenwoordigers goed worden meegenomen in iedere stap. Daarbij werk je nauw samen met collega's binnen én buiten de organisatie. Je werkzaamheden bestaan onder andere uit: - Voeren van oriënterende gesprekken met cliënten en hun netwerk. - Analyseren van zorg- en ondersteuningsvragen en vertalen naar passende zorgmogelijkheden binnen Siloah. - Begeleiden en coördineren van het gehele plaatsingsproces. - Adviseren van cliënten, vertegenwoordigers en verwijzers over zorgaanbod, wet- en regelgeving en financieringsmogelijkheden. - Aanvragen en begeleiden van (her)indicaties. - Samenwerken met coördinatoren zorg, gedragsdeskundigen, zorgkantoren, gemeenten en andere externe partijen. - Deelnemen aan het kernteam Meerzorg en bewaken van het Meerzorgproces binnen de locaties. - Signaleren van ontwikkelingen binnen de gehan...
