Denk jij in mogelijkheden en durf jij initiatief te nemen? Ga dan aan de slag bij het Volledig Pakket Thuis van de thuiszorg in de Waadhoeke en Stiens! Wat ga je doen? Als helpende plus / cliëntondersteuner ga jij ondersteunende werkzaamheden uitvoeren bij de cliënt thuis. Denk hierbij aan het begeleiden, stimuleren, ondersteunen bij en/of overnemen van lichamelijke verzorgende taken zoals het wassen, aankleden, aantrekken van elastische steunkousen, het aanreiken van medicatie en het toezien op inname. Daarnaast onderneem je samen met de cliënt welzijnsactiviteiten die aansluiten bij de behoefte en interesse van de cliënt, waaronder bijvoorbeeld: de krant lezen, spelletjes, wandelen, uitjes, boodschappen doen. De zorg verricht je volgens het zorgplan en op aanwijzing van de cliëntcoach. Je verricht huishoudelijke werkzaamheden, waaronder schoonmaken, de was doen en eventueel koken. Je stimuleert de zelfredzaamheid van de cliënt en neemt over waar nodig. Je signaleert veranderingen bij de cliënt, rapporteert hierover en stemt af met de cliëntcoach. Tijdens je werk kun je ook altijd terugvallen op de cliëntcoach of collega's verzorgende IG van de thuiszorg. De diensten die je werkt zijn 5 tot 8 uur per dienst. We werken met vaste diensttijden, zodat je werktijd niet afhankelijk is van je route. Zodra je rooster bekend is, weet je dus precies waar je aan toe bent! Waar ga je werken? Je gaat aan de slag in het Volledig Pakket Thuis (VPT) -team van Kwadrant in de gemeente Waadhoeke en in Stiens. Het VPT-team zorgt voor mensen die een WLZ-indicatie hebben in de thuissituatie.
