Clientcoach VPT Verpleging Verzorging West-Betuwe 24 - 30 uur 11-10-2026 Komt iemand in aanmerking voor een verpleeghuis, maar blijft hij of zij het liefst thuis wonen? Dan biedt het Verzorgd & Prettig Thuis (VPT) uitkomst. Rivas levert alle benodigde zorg, ondersteuning en verpleging in de vertrouwde omgeving van de cliënt. En daarin speel jij als cliëntcoach een belangrijke rol. Wat ga je doen? Als cliëntcoach binnen het VPT bied en coördineer je de zorg aan cliënten in de thuissituatie. Daarbij staat het behouden of verbeteren van het functioneren, in relatie tot de kwaliteit van leven, centraal. Je voert intakegesprekken met nieuwe cliënten, brengt het netwerk in kaart en onderhoudt contact met familie en mantelzorgers. Ook signaleer je of de gestelde indicatie nog aansluit bij de zorgbehoefte van de cliënt. Binnen het VPT-team heb je een coachende rol, gericht op het leveren van de best mogelijke zorg. Je bent nauw betrokken bij het plannen en roosteren en organiseert overleggen waarin cliënten worden besproken en gezamenlijke afspraken worden gemaakt. Daarnaast denk je actief mee over de best passende invulling van het VPT-pakket, waarbij je goed inspeelt op de wensen en behoeften van cliënten. Je onderhoudt contact met externe zorgverleners, zoals huisartsen, paramedici en de gemeente, om de zorg optimaal af te stemmen. Tot slot werk je aan de zichtbaarheid van het VPT binnen je werkgebied. Je stemt regelmatig af met collega's uit andere VPT-teams om kennis uit te wisselen en het VPT verder te ontwikkelen. Ook coördineer, ondersteun en begeleid je medewerkers Zorg & Welzijn. Hoe ziet mijn team eruit? Als Clientcoach werk je het team de regio van Lingewaal, Geldermalsen en Culemborg.
