Over het Kruispunt

Het Kruispunt is een samenwerkingsverband tussen werkingen in de eerste, tweede en derde lijn, welzijn en gezondheidszorg. De samenwerking heeft als doel om versnippering van de zorg tegen te gaan en een éénduidige, betaalbare en laagdrempelige doch kwaliteitsvolle toegang tot mentaal welzijn te faciliteren. Het Kruispunt richt zich op iedereen ouder dan 16 jaar met emotionele, psychosociale, psychische en psychiatrische hulpvragen.

Om burgers en professionals te ondersteunen in de zoektocht naar gepaste hulp bij psychische kwetsbaarheid richten we toegankelijke onthaalpunten op in elke eerstelijnszone. Het kruispunt focust op volgende opdrachten:

- consult aan verwijzers; - screening en vraagverheldering vanuit een biopsychosociaal kader met de nadruk op vroegdetectie en –interventie met desgevallend gepaste verwijzing; - oriëntering en gerichte doorverwijzing; - kortdurende eerstelijns en doelgerichte psychische en psychosociale begeleiding;

- multidisciplinair overleg.

Momenteel zijn er reeds 2 Kruispunten operationeel. In de toekomst komen er hier nog 3 bij.

Elk Kruispuntteam is samengesteld uit medewerkers uit verschillende partnerorganisaties zoals CAW, CGG, mobiele teams, PAAZ, verslavingszorg, lokale besturen en geconventioneerde zelfstandigen vanuit de conventie ELP.

De Kruispuntcoördinator heeft een netwerkfunctie en ondersteunt de Kruispuntteams. Hij/zij werkt heel nauw samen met de netwerkcoördinator Emergo en directies van partnerorganisaties.

Wat zijn je taken?

- Je vertaalt strategische lijnen naar een concrete werking - Je bewaakt en ondersteunt de dagelijkse werking van elk Kruispuntteam en de medewerkers - Je stroomlijnt en aligneert de praktijken van de verschillende Kruispuntteams - Je geeft mee vorm aan een kruispuntenbeleid, ook op Vlaams niveau - Je neemt een trekkende rol op in de geplande opstart van nieuwe Kruispunten, in samenwerking met nieuwe en vaste partners - Je werkt een opleidingsbeleid uit en bewaakt een kwaliteitscultuur in de werking

- Je neemt graag verantwoordelijkheden op - Je communiceert vlot en graag; je bent een echte netwerker en bruggenbouwer - Je neemt initiatief en hebt aandacht voor medewerkers en de operationele werking - Je hebt een helikopterview en bekijkt zaken vanuit verschillende perspectieven en kan deze met elkaar verbinden - Kennis van, ervaring of sterke interesse in de sectoren van geestelijke gezondheidszorg en welzijn is een meerwaarde - Je bent flexibel en bereidt om verplaatsingen te doen binnen het werkingsgebied Emergo
