Je combineert zorgzaamheid met commercieel inzicht vanuit de regio Rotterdam. Wie zijn wij? Saar aan Huis is een landelijke organisatie die mantelzorgers ondersteunt. Onze missie? Mensen met liefde en respect helpen wanneer het thuis even niet meer lukt. Denk aan ondersteuning voor ouderen, hulp in het gezin bij ziekte of tijdelijke uitval, of gewoon een luisterend oor. Wij werken met betrokken zorgverleners -de Saars en Brammen- die cliënten helpen om zo lang en prettig mogelijk thuis te wonen. Onze organisatie groeit. Daarom zoeken we een servicegerichte en empathische collega die met ons meebouwt aan nog betere mantelzorgbemiddeling. Wat ga je doen? Als Coördinator Mantelzorgbemiddeling: - Ben je het aanspreekpunt voor cliënten, Saars/Brammen en verwijzers - Ondersteun je de Regiomanager op het gebied van relatiebeheer en met het opvolgen van nieuwe klanten - Inventariseer je telefonisch de zorgbehoefte van (potentiële) cliënten en geef je passend advies - Zorg je samen met de Regiomanager voor een perfecte match tussen cliënt en Saar/Bram - Plan je kennismakingsgesprekken met zowel cliënten als Saars/Brammen, en coördineer je het proces tot aan de start van de zorg - Lever je input...
