Als Coördinator POH-GGZ bij Dimence Groep speel je een sleutelrol voor de samenwerking tussen huisartsenpraktijken, praktijkondersteuners en de organisatie. Je bent zowel organisatorisch als inhoudelijk betrokken en draagt actief bij aan de verdere ontwikkeling van de POH-GGZ dienstverlening in onze regio. Wie zijn wij? Dat doen we niet alleen binnen onze eigen organisatie, maar juist ook samen met partners in het zorgveld. Een belangrijke partner daarin zijn de huisartsenpraktijken in onze regio. Via onze praktijkondersteuners huisarts GGZ (POH-GGZ) bieden we laagdrempelige psychologische ondersteuning dicht bij de patiënt. Hierdoor ontvangen mensen sneller passende hulp op een vertrouwde plek. Wat ga je doen? Als Coördinator POH-GGZ ben je verantwoordelijk voor het ontwikkelen, organiseren, versterken van de samenwerking tussen Dimence Groep, huisartsenpraktijken en praktijkondersteuners. Je combineert organisatiekracht met visie. Je signaleert ontwikkelingen binnen de eerstelijnszorg, vertaalt deze naar kansen voor de organisatie en draagt bij aan de verdere positionering van de POH-GGZ.
