Ben jij onze nieuwe coördinerend begeleider? Functieomschrijving Als coördinerend begeleider ben jij de spil rondom de cliënt. Je coördineert de begeleiding binnen je eigen caseload en onderhoudt actief contact met familie, behandelaren, SPV'ers, GGZ-instellingen en andere betrokken samenwerkingspartners. Samen met de cliënt stel je een begeleidingsplan op en zorg je als coördinerend begeleider voor een gestructureerde uitvoering hiervan. Je ondersteunt cliënten bij het vergroten van hun zelfredzaamheid en biedt waar nodig ondersteuning bij praktische zaken, zoals financiële en materiële middelen. Daarnaast verricht je, indien nodig binnen de kaders van de Wlz, persoonlijke zorg en verpleegtechnische handelingen. Je draagt bij aan een veilige en prettige leefomgeving en zorgt voor een zorgvuldige verslaglegging in cliënt- en dagrapportages. Naast je werkzaamheden als coördinerend begeleider binnen je caseload draag je bij aan een positieve werksfeer. Je ondersteunt begeleiders inhoudelijk, denkt mee bij complexe situaties en draagt actief bij aan het bewaken en verbeteren van de kwaliteit van begeleiding. Jouw werkplek De BW+ is onderdeel van beschermde woonvorm de Lorentzweg in Bussum. We bieden begeleiding en ondersteuning in de ADL aan 26 bewoners vanaf ongeveer 70 jaar. De bewoners hebben een psychiatrische achtergrond, een aantal hebben een somatische zorgvraag. Je komt te werken binnen een betrokken en ervaren team van ongeveer 26 medewerkers bestaande uit (coördinerend) begeleiders, meewerkend begeleiders, huishoudelijke ondersteuners en activiteitenbegeleiders.
