Op Groot Schutterhoef in Leusden ondersteun jij mensen met een ernstig meervoudige beperking (EMB) bij hun dagelijkse zorg en welzijn. Tegelijk ben jij degene die overzicht houdt, lijnen uitzet en zorgt dat de begeleiding rondom bewoners goed op elkaar aansluit. Groot Schutterhoef ligt in een groene en rustige omgeving tussen Leusden en Amersfoort. Op deze locatie wonen 20 bewoners met een ernstig meervoudige beperking, verdeeld over twee woningen. Iedere woning heeft een eigen, hecht team. Je komt terecht in een betrokken groep collega's die dagelijks met aandacht, rust en deskundigheid het verschil maken. Binnen deze setting werk je met bewoners die volledig afhankelijk zijn van jouw begeleiding. Communicatie verloopt vaak non-verbaal. Dat betekent dat je kijkt naar kleine signalen zoals houding, spierspanning, ademhaling, mimiek en geluiden. Juist daarin zit de basis van goede zorg en begeleiding. Je werkt volgens de LACCS-visie. Je kijkt naar wat iemand nodig heeft op het gebied van lichamelijk welzijn, alertheid, contact, communicatie en een zinvolle daginvulling. Als coördinerend begeleider zorg je niet alleen dat deze visie in de dagelijkse begeleiding zichtbaar wordt, maar ook dat afspraken en begeleidingslijnen goed worden vastgelegd en uitgevoerd. Jouw rol als Coördinerend Begeleider EMB Als coördinerend begeleider combineer je dagelijkse zorg en begeleiding met regie op het zorgproces. Je staat op de groep en bent dichtbij de bewoners, maar je hebt daarnaast een extra rol in de afstemming, inhoud en kwaliteit van zorg. Je ondersteunt bewoners bij intensieve ADL, maaltijden en dagelijkse verzorging. Je creëert rust, structuur en een veilige omgeving. Tegelijk kijk je overstijgend naar wat bewoners nod...
