Dan is dit jouw plek als coördinerend begeleider VB & Ouderen! Jouw werkplek Op locatie Alfred Tepepad 2 in Driebergen wonen ouder wordende mensen met een verstandelijke beperking. Door het ouder worden kan de behoefte aan begeleiding en zorg veranderen. Bewoners kunnen bijvoorbeeld meer medische ondersteuning nodig hebben of ander gedrag laten zien. De locatie ligt centraal in een woonwijk. We werken vanuit belevingsgerichte zorg , waarbij aansluiten bij de beleving, mogelijkheden en behoeften van de bewoner centraal staat. Jouw team Als Coördinerend Begeleider VB & Ouderen werk je samen met je collega's aan passende begeleiding en zorg. Samen kennen jullie de bewoners goed en bespreken jullie veranderingen in gedrag, gezondheid en ondersteuningsbehoefte. Je werkt samen met je collega's aan goede en passende ondersteuning voor de bewoners. Samen bespreek je wat je ziet, stem je de begeleiding af en zorg je voor een duidelijke structuur. Als coördinerend begeleider heb je daarbij een belangrijke rol. Je signaleert veranderingen, denkt vooruit en bespreekt met collega's wat nodig is. Je werkt methodisch en reflecteert op je eigen handelen en dat van het team. Jouw uitdaging Als coördinerend begeleider VB & Ouderen combineer je dagelijkse begeleiding en zorg met een coördinerende rol. Je biedt structuur en ondersteunt bewoners bij persoonlijke verzorging, dagelijkse activiteiten en een prettig dagritme. Je kijkt naar wat iemand zelf kan en let goed op veranderingen in gezondheid, gedrag en zorgbehoefte. Je werkt met het ECD , ondersteunt bij medicatie en houdt rekening met epilepsie .
