Coördinerend verpleegkundige somatiek - Werken bij Uren per week: 12 uur Jouw verpleegkundige kennis inzetten op een plek waar collega's én andere zorgprofessionals op jouw expertise rekenen? Als coördinerend verpleegkundige somatiek ondersteun je onze woonteams bij somatische zorgvragen van cliënten met een verstandelijke beperking. Wat ga je doen? Je kijkt mee bij complexe situaties, adviseert collega's en brengt verschillende professionals rondom een cliënt bij elkaar. Tegelijkertijd denk je vanuit jouw vak mee over onderwerpen als medicatieveiligheid, palliatieve zorg en de Wet BIG. Je doet dat niet alleen. Je maakt deel uit van ons multidisciplinaire zorgteam, waarin verschillende expertises samenkomen. Naast jou bestaat dit team uit twee andere coördinerend verpleegkundigen somatiek, een toegepast psycholoog, drie gedragswetenschappers, een coördinator kwaliteit en een beleidsadviseur zorg. Je hebt dus collega's met wie je inhoudelijk kunt sparren en samenwerkt bij vraagstukken die meerdere vakgebieden raken. Een zelfstandige én collegiale functie voor 12 uur per week, grotendeels tijdens kantoortijden. De woensdag is in ieder geval een vaste werkdag. Over de verdeling van de overige uren maken we graag afspraken die passen bij de functie én bij jou. Onze cliënten worden ouder en somatische zorgvragen worden steeds complexer. Als coördinerend verpleegkundige somatiek help jij onze teams om daar goede en passende zorg bij te bieden. Je wordt ingeschakeld wanneer een team behoefte heeft aan verpleegkundige expertise. Je brengt de situatie in kaart, denkt mee over wat nodig is en adviseert en coacht medewerkers. Daarbij kijk je niet alleen naar de medische of verpleegkundige kant, maar ook naar wat past bij de cliënt en zijn of haar leven. Daarnaast: - ondersteun en coach je teams bij somatische en verpleegkundige vraagstukken; - signaleer je gezondheidsrisico's en ontwikkelingen bij cliënten; - geef je gevraagd én...
