Als Coördinerend Verpleegkundig Thuismonitoring combineer je directe patiëntenzorg met een overstijgende rol binnen het team. Je bent een van de kartrekkers van de verdere ontwikkeling van thuismonitoring en naast je patiëntgebonden werkzaamheden heb je een spilfunctie in projecten, zorgpaden en innovatie. Een groot deel van je tijd ben je bezig met het monitoren en begeleiden van patiënten op afstand. Je analyseert en interpreteert binnenkomende, soms complexe gezondheidsgegevens. Met je klinische blik en specialistische verpleegkundige kennis voer je op afstand triage uit, bepaal je de urgentie en zet je de juiste interventie in. Je bespreekt je bevindingen met de verpleegkundig specialist of arts en adviseert, waar nodig, over passende vervolgstappen of aanpassingen in het behandel- en/of medicatiebeleid. Je coacht patiënten en hun naasten bij het uitvoeren van metingen en geeft duidelijke uitleg over hun behandeling en gezondheid. Zo help je patiënten om zelf de regie te nemen en draag je eraan bij dat zij langer thuis kunnen blijven of eerder naar huis kunnen na een opname.
