Zorgbalans - Coördinerend verzorgende IG - uren in overleg - Sonnevanck - Hillegom VMBO 0 - 32 Hillegom, Noord-Holland, Nederland Solliciteren Printen Coördinerend verzorgende IG Hillegom parttime / fulltime Specialistisch geriatrisch behandelcentrum Sonnevanck In april 2024 zijn de deuren geopend van Sonnevanck, een specialistisch geriatrisch behandelcentrum van Zorgbalans in Hillegom. Het biedt specialistische behandeling in een veilige omgeving aan mensen in crisis met cognitieve stoornissen en probleemgedrag, met als doel hen dusdanig te stabiliseren dat ze hun bestaan op een andere passende woonplek voort kunnen zetten. Wat doe je als coördinerend verzorgende IG bij Sonnevanck? Als coördinerend verzorgende IG ben jij onderdeel van het behandelteam en lever je input vanuit jouw expertise. Je zorgt dat de juiste zorg en welzijn geboden wordt aan de bewoners in samenspraak met naasten en met het zorgteam. Zo stel je als coördinerend verzorgende IG zorgleefplannen op, houd je ze actueel, coördineer je de zorg en werk je mee in de dagelijkse zorgverlening. Op je afdeling werken verpleegkundigen, verzorgenden en helpenden. Je komt te werken in één van de specialistische teams: 1. Acute opname voor mensen met cognitieve stoornissen vanuit de thuissituatie 2. Afdeling voor interventieopname voor mensen met een dementie en zeer ernstig probleemgedrag (D-ZEP) 3. ELV - gesloten afdeling; voor observatie en screening 4. Langdurige behandelafdeling voor mensen die door hun gedrag niet in een reguliere groep kunnen wonen Heb jij het profiel en de opleiding die we zoeken? - Je hebt het diploma Verzorgende 3 IG en het certificaat Eerst Verantwoordelijke Begeleider (EVB)/ Eerst Verantwoordelijk Verzorgende (EVV). - Je hebt affiniteit met de doelgroep van Sonnevanck (mensen met cognitieve stoornissen en probleemgedrag) - Je bent daadkrachtig en een van jouw talenten is dat je je goed kunt inleven in anderen - Coördineren ...
