Als Coördinerend Verzorgende IG in de thuiszorg van Tellus heb jij oog voor dat hele plaatje. Je bent er voor cliënten in Den Helder en Julianadorp en ondersteunt hen in hun eigen vertrouwde omgeving. Je helpt hen om zo lang mogelijk prettig en veilig thuis te blijven wonen. Tegelijkertijd kijk je verder dan de zorg van dat moment: je stemt af, denkt mee en helpt het team om samen de beste zorg te bieden. Een rol met aandacht voor mensen én ruimte om echt van betekenis te zijn. Waarom werken in ons Thuiszorgteam? Ons thuiszorgteam staat voor persoonlijke aandacht, kwaliteit en samenwerken. We kennen elkaar, zoeken elkaar makkelijk op en kijken samen hoe we de zorg steeds verder kunnen verbeteren. Daar zijn we trots op. Onze thuiszorg is PREZO-gecertificeerd en volop in ontwikkeling. Daarom zoeken we een collega die graag verantwoordelijkheid neemt, kritisch durft te kijken en het leuk vindt om mee te bouwen aan de toekomst van onze thuiszorg. Wat ga je doen? Als Coördinerend Verzorgende IG werk je met je eigen cliënten in de wijk en bied je zorg in hun vertrouwde omgeving. Je: - verleent zelfstandig persoonlijke zorg aan cliënten; - signaleert veranderingen in de zorgvraag en rapporteert deze duidelijk; - stemt af met collega's en de vaste wijkverpleegkundige; - adviseert wanneer aanvullende beoordeling, opschaling of overdracht nodig is; - denkt actief mee over hoe de zorg beter georganiseerd kan worden; - neemt deel aan werkgroepen en deelt nieuwe kennis met je team. Je staat dus dicht bij de cliënt, maar hebt ook een belangrijke rol in de afstemming en kwalit...
