De verantwoordelijkheid dragen, een oprecht dankjewel of een glimlach ontvangen, geeft jou als coördinerend verzorgende in de thuiszorg energie. Samen met je collega's zorg je dat cliënten zich gezien en thuis voelen en ga je voldaan naar huis. Wat ga je doen? Als coördinerend verzorgende in de wijkverpleging ga je aan de slag in de regio Venlo . Je bent een belangrijke schakel in de zorg rondom jouw cliënten: je coördineert de dagelijkse zorg, verleent zelf zorg en verpleegkundige handelingen en geeft deskundig advies waar nodig. In deze functie: - Coördineer je de zorg rondom cliënten , stel je samen met de cliënt en het sociale netwerk zorgplannen op en bewaak je de voortgang van de afgesproken zorg. - Verleen je verzorgende en verpleegkundige zorg , zoals medicatietoediening, injecties en wondzorg, en signaleer je veranderingen in de gezondheidssituatie van cliënten. - Stimuleer je de zelfredzaamheid van cliënten en draag je bij aan een veilige, prettige en vertrouwde thuissituatie waarin de zorg optimaal aansluit bij de actuele zorgbehoefte. Signaleer je tijdens evaluaties dat een zorgplan moet worden aangepast? Dan overleg je met de wijkverpleegkundige en zorg je ervoor dat de zorgverlening blijft aansluiten op de medische noodzaak en wensen van de cliënt. Bovendien werken we met 8-uursdiensten , waardoor je goed aan je contracturen kunt komen. Niets is mooier dan zien hoe cliënten zich dankzij jouw inzet weer fijner, rustiger en zelfstandiger voelen.
