- Over MTB - Rondleiding Consulent Annex BV is het re-integratiebedrijf van de gemeenten Eijsden-Margraten, Gulpen-Wittem, Maastricht, Meerssen, Vaals en Valkenburg aan de Geul. Annex levert haar diensten voornamelijk in opdracht van Sociale Zaken Maastricht Heuvelland. Annex werkt met haar deelnemers aan het ontdekken van hun talenten, het schetsen van een reëel arbeids-marktperspectief, de ontwikkeling van competenties en de realisatie van doelen. Met een team van professionals geven we dit vorm door de inzet van diverse diagnostische instrumenten waaronder (medische) beoordelingen, (psychologische) testen en trainingen. Genoemde instrumenten worden ingezet terwijl de deelnemer aan het werk is in een bedrijfsmatige omgeving. De bevindingen worden vastgelegd in een rapportage welke de basis vormt voor verdere stappen op weg naar invulling van persoonlijk perspectief. 32-36 uur Maastricht Informatie Wegens uitbreiding van ons team zijn op zoek naar een energieke: Consulent 32-36 u p/week Als consulent diagnostiek voer je de regie over de trajecten van deelnemers. Je zet de deelnemer in zijn kracht, weet hem te activeren en te stimuleren. Samen onderzoek je de arbeidsmogelijkheden. Met oog voor eventuele beperkingen breng je in kaart wat voor hem de juiste werkomgeving is en welke randvoorwaarden nodig zijn. Dit alles doe je zodanig dat voldaan wordt aan de vraag van de opdrachtgever. De werkzaamheden: - Houden van intakegesprekken met deelnemers met als doel het verzamelen van relevante informatie ter beoordeling van de (her)plaatsingsmogelijkheden, teneinde een perspectiefplan op te stellen (i.s.m. ketenpartners); - Uitvoeren en begeleiden van een individueel trajectplan; - Bemiddelen naar en acquireren van praktijkdiagnose plekken; - Controleren van de voortgang en zorgdragen voor het plaatsingsproces middels evaluatiegesprekken; - Inzetten van de benodigde diagnostische instrumenten en terugkoppelen aan deelnemer en/o...
