In de rol van consulent wmo heb je een afwisselend takenpakket: - Uitvoeren van vraagverheldering en beoordeling van aanvragen Wmo met name hulp bij het huishouden, vervoersvoorzieningen, rolstoelvoorzieningen, woonvoorzieningen en eventueel begeleiding bij voorkeur gedaan middels huisbezoek(en); - Onderzoek doen met als doel zorgen voor de juiste hulp en/of passende (maatwerk)voorziening(en); - Beslissen over de in te zetten ondersteuning (inclusief de eigen mogelijkheden en de mogelijkheden in het sociale netwerk en algemene voorzieningen) in het kader van wet- en regelgeving en dit onderbouwen; - Leggen van verbanden wat ten goede komt aan de integrale dienstverlening aan de cliënt; - Proactief schakelen met interne collega's en / of andere externe instanties bij problematiek op andere gebieden. Als consulent Wmo werk je op de afdeling Zorg en Ondersteuning. Vanuit deze afdeling wordt ook het leerlingenvervoer en de Wmo begeleiding uitgevoerd. Het is mogelijk dat in de loop van de tijd het takenpakket wordt uitgebreid met Wmo begeleiding. Met wie ga je werken? Je wordt onderdeel van de afdeling Zorg en Ondersteuning. Het team WRV bestaat uit 10 medewerkers. Je werkt veel samen met collega's in het Huis van de Stad. Denk daarbij aan juridische medewerkers, medewerkers applicatiebeheer, senior medewerkers, medewerkers administratie, medewerkers backoffice, consulenten begeleiding, consulenten Jeugd, Inkomen en Werk en de afdeling beleid. Externe contacten heb je uiteraard met inwoners, maar ook met externe indicatieorganen, zorgaanbieders, voorliggende voorzieningen, vervoerders, ziekenhuis en het sociaal team. Wat heb jij in huis?
