Als Consulent Wmo werk je aan oplossingen die passen bij iemands persoonlijke situatie. Je krijgt een afwisselende rol met directe betekenis voor zelfredzaamheid en participatie. Je behandelt meldingen integraal: je onderzoekt de hulpvraag, bespreekt eigen mogelijkheden en betrekt het netwerk, algemene voorzieningen en kernpartners. Bij complexe situaties voer je regie en lever je inhoudelijke expertise aan collega-consulenten. - Inventariseer de brede problematiek en ondersteuningsbehoefte. - Indiceren en verstrekken van passende Wmo-maatwerkvoorzieningen. - Voer gesprekken met inwoners en leg afspraken zorgvuldig vast. - Beoordeel voorliggende, collectieve en algemene voorzieningen. - Adviseer consulenten en werk samen met keten- en kernpartners. Functie-eisen Je combineert inhoudelijke kennis met een nauwkeurige aanpak en weet complexe vragen te vertalen naar passende ondersteuning. Je hebt ervaring met indicatiestelling en houdt oog voor de inwoner, diens mogelijkheden en de geldende kaders. - Hbo werk- en denkniveau. - 3 tot 5 jaar ervaring als Wmo-consulent of in een vergelijkbare functie. - Aantoonbare ervaring met indiceren van Wmo-maatwerkvoorzieningen. - Kennis van Wmo, Wlz, Zvw en Participatiewet. - Kennis van doelgroepen, ziektebeelden en psychische of psychosociale problematiek.
