Consulent zorg en veiligheid en OGGZ-coördinatie 36u/w Volgnummer: 137549 Publicatiedatum: 28-09-2026 Locatie: ZUIDHORN Contact DC Professionals [email protected] 085 020 1022 Standplaats: ZUIDHORN Duur: 01-11-2026 - 01-02-2027 Optie tot verlenging: Ja Reageren voor: 14-10-2026 • Beoordelen en analyseren van meldingen binnen de Openbare Geestelijke Gezondheidszorg. • Uitvoeren van triages en risicobeoordelingen. • Signaleren van complexe zorg- en veiligheidsvraagstukken. • Opstellen van heldere rapportages en feitelijke samenvattingen. • Afstemmen met gemeenten, politie, woningcorporaties, zorginstellingen en andere ketenpartners. • Bepalen welke organisatie verantwoordelijk is voor het vervolgtraject. • Zorgvuldig verwerken van persoonsgegevens conform de AVG. • Bijdragen aan het verbeteren van werkprocessen, protocollen en handboeken. • Deelnemen aan projectgroepen en overleggen binnen de organisatie. • Verzorgen van voorlichting over de werkzaamheden van de OGGZ. Kandidaatomschrijving • Een afgeronde hbo-opleiding binnen het sociaal domein, de zorg of de GGZ. • Hbo werk- en denkniveau. • Ruime en aantoonbare werkervaring binnen de GGZ, bemoeizorg en/of OGGZ. • Aantoonbare ervaring met triageren. • Gedegen kennis van de sociale kaart van het Westerkwartier. • Kennis van psychische en verslavingsproblematiek. • Actuele kennis van relevante wet- en regelgeving, waaronder de AVG. • Ervaring met procesmatig werken. Functie-eisen: Minimaal opledingsniveau - HBO Minimale werkervaring - Twee jaar relevante werkervaring. Bijvoorkeur in dezelfde functie. Inzet via detachering - De kandidaat wordt uitsluitend aangeboden via detachering en is gedurende de opdracht in loondienst van de inschrijvende organisatie.
