Als Eerst Verantwoordelijke Contactverzorgende (EVC/EVV) binnen Zuyderland Zorgcentra begeleid je de intake van nieuwe cliënten , inventariseer en bespreek je de zorgbehoefte, wensen en lichamelijke mogelijkheden/beperkingen en gewenste leefstijl met cliënten en hun familie of mantelzorgers. Je coördineert het hele proces op basis van het zorgleefplan . Je organiseert evaluaties in multidisciplinair verband en doet eventueel voorstellen voor bijstelling daarvan en borgt de uitvoering.
