Wir suchen ab sofort 1 Dipl. Gesundheits- & Krankenpflege

(m/w/d) 80 % in Frastanz ab sofort In unseren sozialpsychiatrischen Wohnangeboten begleitest du Menschen mit psychischen Erkrankungen auf ihrem Weg der weitestgehenden Verselbstständigung in den Bereichen Lebensgestaltung, Gesundheitsförderung und Alltagsbewältigung.

Dein Aufgabengebiet

Im Wohncoaching begleitest und coachst du Klient:innen ressourcenorientiert und auf Augenhöhe bei der Bewältigung ihres Alltags. Du moderierst Gruppenprozesse und gemeinsame Aktivitäten, um soziale Kompetenzen, Teilhabe und Selbstverantwortung zu stärken. Zusätzlich ist das Medikamentenmanagement ein zentraler Aufgabenbereich: Du organisierst und verwaltest die Medikation für den gesamten Bereich, stellst sie nach

ärztlicher Verordnung zusammen und begleitest Klient:innen beim eigenständigen Richten. Dabei arbeitest du eng mit Ärzt:innen, Apotheken und dem Team zusammen. Zudem bringst du proaktiv Ideen für die strukturelle

Weiterentwicklung ein, um den organisatorischen Rahmen kontinuierlich zu verbessern.

Was du mitbringst

¿ Eine abgeschlossene Ausbildung in der Dipl. Gesundheits- und Krankenpflege

¿ Erfahrung in der Arbeit mit Menschen mit psychischen Erkrankungen und Verhaltensauffälligkeiten

¿ Fähigkeit zum selbstständigen, strukturierten und eigenverantwortlichen Arbeiten

¿ Hohe Belastbarkeit und professioneller Umgang mit Krisen- und Ausnahmesituationen

¿ Teamfähigkeit sowie verlässliche Kommunikations- und Dokumentationskompetenz

¿ Ein hohes Maß an Reflexionsfähigkeit
