Mit dem Pflegenotdienst bieten die Johanniter in Wien ein einzigartiges mobiles Angebot zur Versorgung kurzfristiger Pflegenotfälle. Hier kannst du eigenverantwortlich arbeiten und für andere da sein.

Zur Verstärkung unseres Teams suchen wir 1 Dipl. Gesundheits- und Krankenpfleger:in (Teilzeit nach Vereinbarung)

Der Pflegenotdienst schließt die Lücke zwischen Spitalsentlassung und Langzeitpflege und setzt damit neue Maßstäbe in der mobilen Betreuung. Wir bieten rasche Hilfe und professionelle Pflege und Beratung in Notsituationen - 24 Stunden täglich. Mit Herz und Verstand helfen wir unseren Patient:innen (Klient:innen), den Alltag leichter zu bewältigen.

Deine Aufgaben

* Pflegeinterventionen im Pflegenotfall

* Pflegebedarfserhebungen und Pflegeberatung

* Abklärung von komplexen Pflege- und Betreuungssituationen

* Kooperation mit Notfall- und Einsatzdiensten (Wiener Rettung, ÄFD, Sozialruf Wien, u.a.)

* interdisziplinäres Fallmanagement

* Entlassungsmanagement

Unsere Anforderungen an deine Persönlichkeit

* interdisziplinäres Kommunikations- und Kooperationsvermögen

* Konflikt- und Problemlösungsvermögen

* Innovationsfreude, wertschätzende Umgangsformen

* Lösungskompetenz in Krisensituationen

* Verlässlichkeit und Verantwortungsbewusstsein

Dein Profil

* Diplom in der allgemeinen oder psychiatrischen Gesundheits- und Krankenpflege

* Kenntnisse in gesetzlichen Grundlagen der Pflege

* gute EDV-Kenntnisse

* Berufserfahrung und Kenntnisse im Entlassungsmanagement wünschenswert

* Führerschein B

Das Einstiegsgehalt für diese Position liegt laut Kollektivvertrag der Diakonie bei EUR 3.358 brutto pro Monat (inkl. Pflegezuschuss). Je nach Qualifikation und Berufserfahrung werden wir mit dir in einem persönlichen Gespräch ein faires Gehalt ermitteln.
