Wir suchen ab November 1 Diplomierte Gesundheits- und Krankenpfleger_in als

Community Nurse der Stadt Linz

(25 - 35 Wochenstunden)

Ihr Aufgabengebiet:

* Niederschwellige, stadtteilnahe Anlaufstelle für Senior:innen sowie Angehörige in Fragen von Pflege,

Gesundheit und Unterstützung im Alltag

* Durchführung präventiver Hausbesuche und umfassender Pflegeassessments

* Planung, Umsetzung und Evaluation von Maßnahmen der Gesundheitsförderung, Prävention und Stärkung

der Gesundheitskompetenz

* Information, Edukation und Beratung, insbesondere Vorbereitung auf Pflege- und Betreuungsaufgaben,

Anleitung pflegender Angehöriger sowie Organisation und Durchführung von Schulungs- und

Bildungsangeboten

* Koordination und Vermittlung individuell angepasster Pflege-, Betreuungs- und

Gesundheitsförderungsangebote, inkl. Social Prescribing und Nahtstellenmanagement zu Gesundheits-,

Pflege- und Sozialdiensten

* Nicht in den Aufgabenbereich fallen Tätigkeiten im Rahmen der mobilen Dienste und Hauskrankenpflege!

Was Sie mitbringen:

* Sie haben ein Diplom bzw. einen FH-Abschluss der Gesundheits- und Krankenpflege und mindestens drei

Jahre Berufserfahrung

* Abgeschlossenes Studium der Gesundheits- und Krankenpflege bzw. im Gesundheitswesen von Vorteil

* Sehr gute Kenntnisse und sichere Anwendung des pflegerischen Pflegeprozesses inkl. Assessments auf

individueller, familien- und gemeindebezogener Ebene

* Ausgeprägte Kompetenz in Gesundheitsförderung und Prävention

* Hohe Kommunikations- und Beratungskompetenz

* Erfahrung und Motivation zur interprofessionellen Netzwerkarbeit

* Ausgeprägte Selbstorganisation und Eigenverantwortung, Bereitschaft zu aufsuchender Tätigkeit im

Stadtgebiet

* Kenntnisse in Projekt- und Prozessarbeit
